Important Note

It is important to ask the donor why the test is being performed.  If testing is for employment reasons, ask if the employer has a contract with a testing lab.  If there is a contract the collector needs the account number for that lab which is usually on a request (People Pass) or on a chain of custody paperwork.  Any collection kit may be used and the specimen is sent to the specific lab on the chain of custody.

If the employer does not have an account with another lab use Alere (Escreen) account GPHealth Misc.

If the testing is for a legal agency use  Alere (Escreen) and search the Escreen website for an account for that agency or use GPHealth Misc 

If the testing is for Department of Transportation, the donor needs to be redirected to GPH URGENT CARE. GPH LABORATORY does not offer DOT testing or collections.
